What do I need to bring on the first day?
Do I need a prescription?
   Yes, in order to bill your insurance company you will need a prescription from your medical doctor (MD),  doctor of osteopathy (DO),  nurse practitioner (NP), or physician's assistant (PA).  If you are a self-pay patient not using insurance either a prescription is required or any documentation that you have seen a practitioner for the condition you are being seen for (i.e. x-ray, MRI, visit summary etc)
